Horse Lab Membership Agreement Please read carefully and check each box to confirm agreement. 1. Membership Details *I understand Horse Lab includes: 4 Lessons + 15 Practice Hours OR 8 Lessons (choose one per month). No mixing of practice hours and extra lessons. All sessions must be scheduled by the 1st of the month. 2. Payment *I agree to pay $640/month, due on the 1st of each month. I understand late payments incur a $25 fee after 3 days.3. Scheduling & Cancellations *24-hour notice required for rescheduling lessons only.Practice hours are forfeited if canceled (no refunds/rollovers).No-shows result in full fee forfeiture.4. Liability Waiver *I accept the inherent risks of horseback riding/handling.I release Monica O. Knight/United Mustangs of America from liability for injuries, accidents, or damages.I agree to follow all safety rules and instructor guidance.5. Sanctuary Horse Care *I will treat school horses with respect (no rough handling, unauthorized feeding, or unsafe equipment).6.
